If you're having trouble unlocking this achievement, disable the Anniversary DLC in Steam.
Multiple people have reported unlocking it afterwards.

I was able to unlock it by disabling the DLC and taking out the large intestine from the patient. Afterwards hang it around his neck.
If the Achievement doesn't unlock just keep adjusting the "Scarf".
