To get the 12 policies you will need a mix of government, leaders, wonders and great people.
These are what I found.

Government needed - Democracy = 8 policies
Leader - Pericles (Greece) = +1 Wildcard slot
Great Person - Adam Smith (Merchant) = +1 economic slot
Wonders - Forbidden Palace = +1 Diplomacy slot
- Potola Palace = +1 Wildcard slot
- Allhambra = +1 Military slot
- Big Ben = +1 Economic slot
dabeef2112This is even easier if you have the Gathering Storm expansion. You can choose one of the new 10 policy slot governments (Digital Democracy, Synthetic Technocracy or Corporate Libertarianism) and then you only need 1 Wonder or Adam Smith. It's also possible to vote for a government type to gain a slot but this is a Random event so I wouldn't count on it happening when needed.
