While having the Detect Thoughts spell active during dialogue, and when a prompt labeled [DETECT THOUGHTS] appears, click on it. The achievement will be awarded upon a successful roll.
*** Spoiler - click to reveal ***I can't confirm this but I am pretty sure that if you are talking with someone with a wiggler then you can automatically use detect thoughts. I used it on a goblin while entering the village but I don't have the detect thoughts spell
