As per Doctor's guide on Steam, place exactly three satchels on each of the closed apertures on the ground where the Nihilanth's healing crystals pop out. Destroy its three shield generators with rockets, blow up its glass dome, and shoot some rockets at its big forehead. It will then raise the crystals to heal itself, so when that happens, activate the satchel detonator to get the achievement.
